Homeless Services Case Manager

Description:This is a professional clinical position providing case management, counseling and supportive interventions to individuals and families who are homeless. Work involves the development of self-sufficiency plans and application of psychotherapeutic and prevention practices, with an emphasis on resiliency and asset development. Services are provided in the community.
Duties:1.Establish and maintain effective rapport with consumers.

2.Develop accurate and complete clinical evaluations/assessments of consumers.

3.Develop realistic and effective self-sufficiency/treatment plans with consumers, and update them as appropriate.

4.Maintain continuity of contact with consumers as appropriate to their plans.

5.Maintain appropriate professional boundaries with consumers.

6.Involve significant others of consumers in the activities as appropriate.

7.Exhibit flexibility regarding clinical approach in order to most effectively respond to differing consumer problems.

8.Transfer and/or terminate consumer cases in a professional manner.

9.Adhere to Organization policies, professional/ethical standards and legal requirements regarding clinical practice.

10.Plan and implement psychotherapeutic and psychoeducational groups.

11.Transport clients as needed and required in personal vehicle as part of service provisions.

12.Visit or work at sites outside the office as needed or required for services provision and provide own safe transportation to such locations.
Qualifications:Formal Education

Bachelorís degree in Psychology, Social Work or related field required, with two years experience preferred.

Skills

1.Must verbally communicate in an appropriate and professional manner with consumers and staff.

2.Must effectively apply clinical knowledge.

3.Must communicate ideas in a clear, concise written format and adequately express self both verbally and in writing.

4.Must possess organizational and time management skills.

5.Must maintain an appropriate balance between autonomous functioning and the use of supervisory support.

6.Must possess basic computer literacy.

7.Reading and writing skills that enable reading and writing progress notes, treatment plans and pertinent information concerning the consumer.

Experience/Knowledge

1.Must have sufficient clinical knowledge to provide clinical intervention with adults who are at risk, utilizing various approaches. Familiarity with the resiliency paradigm preferred.

2.Must have minimum one year of case management experience preferably working with the homeless population.

3.Must acquire knowledge of community resources, including housing, homeownership opportunities and support programs.
