FAMILY CARE MANAGER

|Description:||This position is with the Diversion Services specifically under the Eckerd Alternatives Contract to decrease the number of children entering the Child Welfare System. |
|Duties:||The successful candidate will work with families using a team approach to reduce potential removal of children from the home. It includes assessing, linking, coordinating and monitoring services from mental health, social and educational aspects. Collaborate and develop service plans in partnership with the family. Work closely with family to identify services needed in the home to maintain the least restrictive environment. Services are provided in-home or community. Work closely with the family to connect with resources and offer educational, financial or other assistance as needed or appropriate. Attend Integrated Decision Team Meetings. Transport clients in personal vehicle as part of service provision. Complete documentation in a timely manner relative to services provided. Handle crisis situations in a professional and timely manner and apprises supervisor. Based upon the risk factors, the Family Care Manager will fulfill more frequent Weekly visits for families identified as Low Risk, the Care Manager will visit the family in the home at least once a week with all caregivers/parents/children. For families identified as Moderate risk, the Care Manager will visit the family at least three times a week. For families identified as High Risk, the Care Manager will offer to visit at least four weekly visits.|
|Qualifications:||o Bachelorís degree in Psychology, Social Work, Counseling, or closely related Human Services field from an accredited college or university.|
o Two years related experience in mental health or social services.
o One year of the required experience must be with child welfare.
o Must complete all required corporate of industry related training.
o Must have the ability to respond to the needs of families after hours or on the weekends.
o Requires extensive driving.
o Must have and maintain clear driving record, maintain required corporate levels of personal vehicle insurance and must pass safety vehicle inspection check.
o Must have excellent organizational skills, time management skills and be computer literate.
o Experience with electronic records a plus; experience with FSFN preferred.
